Need help applying on Healthcare.gov Cognosante can help! Cognosante is from Madison, but travels to certain areas to set up Enrollment Assistance locations. When Applying for Insurance through the Marketplace you will need: Birthdates and Social Security numbers (or document numbers for lawfully present immigrants) for each person seeking insurance, Employer and income information for every member of your household (i.e. pay stubs, W-2 forms or 2012 tax records), Policy numbers for all health insurance plans currently covering members of your household
