Registered in Sauk Prairie School District
Qualify financially (low income or federal poverty guidelines)
Applications available at:

Or at St. Vincent de Paul Community Store 815 19th St. Prairie du Sac, WI 53578
Applications should be filled out and returned on or by: November 19, 2018.
