Mental Health Therapist - Behavioral Health Services (Full Time)

POSITION SPECIFICS
Title:  Mental Health Therapist
FTE:  Full Time (40 hours/week)
Schedule: Monday-Friday 8:00am-4:30pm
Holiday Rotation:  None
Weekend Rotation: None

POSITION SUMMARY
The Mental Health Therapist primarily collaborates and consults with the local school-based, multi-disciplinary mental health team to address the needs of students and families and provide clinical mental health services to this population. This role provides services that bridge the time between identified need and the start of community-based clinical services. Some services will be ongoing services provided to or on behalf of students or school connected individuals, whose circumstances present a critical level of need. In addition, this role may also perform duties associated with the Behavioral Health department in connection with the clinics/hospital as needed.
 
POSITION TECHNICAL RESPONSIBILITIES

1. Provide diagnosis, evaluation, monitoring, and follow-up services and consultation to students/families/other identified patients.
2. Collaborate and consult with the multi-disciplinary school-based mental health team to address the needs of students and families.
3. Coordinate patent care with schools, families, primary care, other specialty providers as needed.
4. Conduct individual/family/group counseling, utilizing evidence-based practices.
5. Assist in contributing to continuity of care planning as students/families/other patients transition to other community-based services.
6. Maintain progress notes and other documentation required.
7. Work collaboratively with Sauk Prairie Healthcare on projects and initiatives related to Behavioral Health Services.

• Required:  Master’s degree in Social Work, Psychology, or Counseling.
• Preferred:  None

• Required: 2 years of experience providing evidenced-based treatment as a mental health provider. 
• Preferred: 3-5 years of experience providing evidenced-based treatment as a mental health provider to include some experience working with students/families in a school-based setting. Experience working with substance use disorders.

• Required:  Relevant license:  Licensed Clinical Social Worker-LCSW, Licensed Professional Counselor-LPC, Licensed Marriage and Family Therapist-LMFT, or Licensed Psychologist.
