90 Oak St., Prairie du Sac

INTRODUCTION:
This request for proposal (RFP) seeks developer qualifications and proposals for the redevelopment of the Sauk Prairie Healthcare Wellspring Campus (WC) located at 90 Oak Street, Prairie du Sac, WI. The subject property is a 7-acre parcel with a 163,000 square foot former hospital/medical office building.

This document presents the characteristic of the site, it’s context, and general information related to its development potential. The information shall not be treated as inclusive of all data available, but should be considered a reasonable attempt to expose the reader to key elements of existing master plans, codes, and policies related directly and indirectly to the site. This document outlines minimum submission requirements and the basic selection process. The purpose is to identify the best proposals and qualified teams from which to select the ultimate development partner(s).

BACKGROUND:
Sauk Prairie Healthcare (SPH) owns and actively occupies the Wellspring Campus that, until April 2014, served as a community hospital, and until October, 2014 housed three specialty clinics. Hospital services were relocated to a new greenfield hospital site in April 2014 and the specialty clinics were moved to the new campus in October 2014. Since 2014, the Wellspring Campus has housed several support departments, outpatient rehabilitation services, and a wellness-fitness facility. SPH also donates 3,100 sf to the Good Neighbor Clinic, which provides free clinic services to area residents who are unable to pay for care.
